How It Works
- VIP Care Card: As a patient of Hoskinson Health and Wellness Clinic, you’ll automatically receive a VIP Care Card. When presented at a hospital or emergency department, it helps inform your physicians of your relationship with Hoskinson Health. Your card serves as your link to round-the-clock care team, ensuring that your care is coordinated with us, especially if a Hoskinson provider is listed as your primary care physician or if hospitalization is required.
- Seamless Coordination: If you’re admitted to a hospital, whether locally or out of town, our team will make every effort to support your care whenever possible. While treatment decisions remain with the attending hospital team, our providers are available to offer input to help ensure your ongoing care aligns with your medical history and long-term goals.
- Around-the-Clock Physician Access: For urgent health needs outside regular clinic hours, Hoskinson Health provides access to an on-call physicians 24/7 through our overnight answering service. In truly urgent or emergent situations, our hospital-based providers are available to offer timely medical guidance or intervention.

FAQ: VIP Care & Hospitalist Services
Will my Hoskinson provider be notified if I go to the emergency room?
While your VIP Care Card helps identify your connection to Hoskinson Health, we are not automatically notified when you present to an emergency department. We typically receive notice only after the fact, when discharge paperwork is sent. If a Hoskinson provider is listed as your primary care physician, we may receive updates sooner.
What happens if I’m admitted to the hospital?
If you are admitted to Campbell County Health, you can ask to be cared for by our hospitalist team. Only when requested for by the patient, Dr. Natalie Owens-Sloan and Dr. Steven Clements are available to manage your hospital care. Asking for them by name ensures they can be assigned to manage your care throughout your stay.
After discharge, your care continues seamlessly: you’ll either follow up with the same hospitalist at our clinic, be referred to a specialist, or reconnect with your primary care provider, who receives a full report from the hospitalist who treated you.
What if I’m transferred to another hospital?
If you are transferred to another hospital (e.g., Banner Health in Casper, UCHealth in Colorado, Monument Health in Rapid City, or Billings Clinic), our hospitalist nurse may be able to continue following your case. If we're notified and have access to that facility’s records, we upload records into your Hoskinson chart and help coordinate any necessary follow-up or referrals.
Does it matter which provider admits me?
If you are admitted by a Hoskinson hospitalist at CCH, your care is more integrated. Having direct access to your medical history, prior test results, and care team notes, we can any avoid duplicate testing, medication errors, and delays. Continuity of care is preserved from admission through post-hospital follow-up.
Is someone available to help after hours?
Yes. Our 24/7 answering service can connect you to an on-call provider when needed. However, this access is meant for urgent or emergent health concerns only. If your concern is not urgent (e.g., medication refills, chronic issues, or medical equipment needs), your call may be scheduled for follow-up during normal clinic hours.
Examples of non-urgent concerns that may wait until morning include:
- Mild constipation
- Requests for CPAP equipment
- Questions about routine medications
Can I get a diagnosis or treatment over the phone?
While our physicians can offer general guidance, they cannot diagnose conditions or prescribe treatment over the phone without a proper medical evaluation. If your symptoms are serious, unclear, or worsening, you may still need to visit the emergency room for a full assessment.
